OUR PROGRAMS & SERVICES...

At ACC, we provide a broad suite of services to help individuals build the lives they choose. Our only eligibility requirement is identifying with a disability; no medical paperwork or “proof” required.

**All of our services are accessible in the language most natural to each individual, including American Sign Language for Deaf and Hard of Hearing community members

Programs:

Housing Navigation to help people find and maintain affordable and accessible housing

Personal Assistance Services:  support with cleaning, cooking, bathing, and more, through options like In-Home Supportive Services (IHSS) or our Personal Assistant Registry.

Peer Support Services provide understanding and encouragement from staff and board members with lived disability experience. In addition

ACC helps people obtain and use Assistive Technology, supporting everything from mobility tools to communication devices. Our programs also serve individuals who are blind or have low vision

Community Transition Services: help people move from institutional settings back into community living.

Youth Transition Program:  supports youth with disabilities ages 14-26 as they work toward adulthood. Services are provided based on individualized goals and focus on a wide range of independent living topics. This includes, but is not limited to, pre-employment services (resume, job application, mock interview prep), independent living skills training (cooking, cleaning, learning about accessible transportation options), housing navigation, benefits assistance, information and referral (additional youth services, events, forums, or other services that might best fit your needs), and more!

We are committed to providing accessible services to youth with disabilities. We do not want transportation to be a barrier to our services; therefore, we can meet via Zoom/Google Meet, in the community, or in our office. In addition to our one-on-one support, we are also happy to connect with schools or collaborate with other organizations to provide clear and consistent services to mutual clients.
